Residents are invited to print this form using their Web browser, complete and sign it, and then mail it with payment to: Continuing Education, Horace Greeley High School, Chappaqua, NY 10514.

Register Early!  Courses will be filled on a first-come, first-served basis.  Courses with insufficient registration will be cancelled and fees refunded.  Refunds will not be granted under any other circumstances.   Unless your check is returned, registration is automatically accepted.  Make check payable to Board of Education. Mail to: Continuing Education, Horace Greeley High School, Chappaqua, NY 10514.  Please use a facsimile if additional forms are needed.
